Getting Help When You Need It
The Employee Assistance Program (EAP) is a confidential, professional counseling and referral service designed specifically for employees and their family members. Your EAP is an employee benefit offered by Daybreak Behavioral Health, a service of Florida Hospital Heartland Division.
Your EAP is available when you need help. A simple phone call is all you need to start the process for you and your family. Your EAP has offices conveniently located near your home or where you work. Therapy is available to provide you with counseling to clarify your concerns identify solutions and develop a plan for solving your problems.
Here’s how we can help. The EAP Counselor can help resolve problems that affect your personal life or job performance.
These problems may include:
- Stress
- Marital/Relationship Issues
- Family/Parenting Issues
- Anxiety
- Depression
- Elder Care Issues
- Grief and Bereavement
- Substance Abuse
- Other Personal Problems
Our Commitment: Utilization of EAP Services
As an employee, you or your family members will be allowed a number of sessions with the EAP counselor to assess the problem. Often, the problem is resolved within the EAP time frame. If further counseling is recommended or requested, you or your family member will be referred to the provider best equipped to provide the assistance you may need.
Commitment to Confidentiality
EAP counseling is confidential, subject to State of Florida law. Counseling discussions between you and the EAP counselor, and your use of EAP services, will not be released to anyone without your written consent. This right of privacy is maintained for family members as well as the employee.
Cost
There is no cost to employees and family members who use EAP services. If you are referred for additional counseling, any fees or co-payments would be your responsibility. You will be advised of your company medical benefits coverage for those services. Every effort will be made to find the best resource for you.
EAP Referrals
The Employee Assistance Program works with employees on personal problems and/or problems affecting work performance.
Referrals can be made in two ways:
- Self Referral – Any employee or family member can simply call the EAP office for information or to make an appointment
- Employer Referral - Supervisors may suggest to an employee that he/she seek help when thee is a noticeable change in the employee’s work performance.
